1. INITIAL EVALUATION
Beauty Surgery Guide evaluates all prospective patients to ensure safety and candidacy. Patients must disclose comprehensive medical information and history.

Evaluation Fee: A fee of $500.00 is required at the time of contract execution.

Terms: This fee is earned upon receipt and is non-refundable.

Validity: This contract remains valid for 24 months. During this period, your quoted price is guaranteed, and a one-time courtesy rescheduling is included.

Right of Refusal: After evaluation, the surgical team may determine a patient is not a suitable candidate. Beauty Surgery Guide reserves the right to decline any procedure. In such cases, the Evaluation Fee remains non-refundable.
2. METHODS OF PAYMENT
Patient accepts responsibility for all “Procedure Fees” identified in the service description.

Payment Schedule: Procedure Fees must be paid in full at least 1 day prior to the scheduled surgery.

Installments: Patients may make payments toward their balance via our secure portal (minimum payment of $1,000 or the remaining final balance).

Accepted Forms: Cash (USD), money orders, cashier’s checks, and major credit cards (Visa, MasterCard, Discover, Amex). The cardholder must be present with valid, non-expired identification.

Financing: We accept options such as United Medical Credit, Alphaeon, CareCredit, and PatientFi.

Note: Contract prices are “cash prices.” Financing may incur surcharges (0%–25%) depending on lender terms.

Emergency Refunds: In the event of a documented medical emergency authorized by management, a partial refund (not exceeding 50% of the deposit) may be granted to the original payment method.
3. FACILITY ROLE & INDEPENDENT PHYSICIANS
Beauty Surgery Guide operates as a surgical facility.

Limited Role: The facility provides the infrastructure, coordination, and support staff. We do not provide medical advice or medical services directly.

Independent Contractors: All surgeons are independent contractors. The Physician is solely responsible for your medical care, surgical decisions, and post-operative management.

Physician Availability: If your preferred physician becomes unavailable, we will attempt to provide another qualified surgeon. Alternatively, you may reschedule within 12 months of the original date.
4. NO EXPRESS OR IMPLIED WARRANTIES
Medicine and surgery are not exact sciences.

Outcome: Payment is for the performance of services; there is no guarantee of a specific aesthetic result or outcome.

Variables: Results depend on individual biological factors, such as tissue response and wound healing.

Secondary Procedures: If a “touch-up” or secondary procedure is suggested by your physician, additional fees (Surgeon, OR, and Anesthesia) may apply.
5. PATIENT RESPONSIBILITIES

Medical Clearance: Patients are responsible for the cost and coordination of their own medical clearance and lab tests. Results must be received by the facility no later than 10 days prior to surgery. Failure to do so may result in a 12-month rescheduling delay.

Travel & Lodging: Beauty Surgery Guide is not responsible for any third-party travel or accommodation expenses under any circumstances.

Surgical Criteria: Physicians maintain strict requirements regarding BMI, nicotine/tobacco use, and drug/alcohol abstinence. If a Physician declines to proceed due to a patient failing these requirements, the Procedure Fees (excluding the deposit/evaluation fee) may be refunded or the surgery rescheduled.

Aftercare Arrangements: You must consult with your coordinator 10 days prior to surgery regarding post-op care. Failure to have a registered nurse or approved aftercare plan in place may result in a surgical delay.
6. APPOINTMENTS & PRE-OPERATIVE VISITS

Arrival: Patients must be at the facility at least one day prior to surgery for paperwork and prescriptions.

Day of Procedure: Arrive at least 1 hour early with a designated “Caretaker” (18+ years old).

Transportation: Rideshare services (Uber/Lyft) and Taxis are strictly prohibited for post-surgical transport. You must have a reliable, known caretaker to take you home.
7. DRUG TEST & SAFETY CONSENT
All patients agree to a urine analysis drug test.

Refusal: Results in immediate withdrawal from the surgical schedule.

Positive Results: Testing positive for recreational drugs, nicotine (if prohibited by surgeon), or pregnancy will result in immediate cancellation.

Fees: A $1,000 Cancellation Fee applies in these instances.
8. GOVERNING LAW & JURISDICTION
This Agreement is governed by the laws of the State of Florida. Any legal proceedings shall be handled exclusively in the state courts of Miami-Dade County, Florida.
9. MODIFICATIONS & FINALITY

Entire Agreement: This document, along with the Refund/Cancellation policy, constitutes the full agreement. Verbal promises not contained herein are not binding.

Deposits: Initial non-refundable deposits are non-transferable under any circumstances.

Legal Fees: In the event of litigation, the prevailing party is entitled to recover reasonable attorney’s fees and costs.
By executing this contract, the Patient acknowledges they have read, understood, and agreed to all terms listed above.
